Job Description
Job Summary: Provides professional nursing care and other supports to children with complex care needs who live in the community. Promotes the wellbeing of the children served. This position may involve working up to 8hr days and/or evenings/and weekends.
Reports To: Resource Ability Nursing Supervisor
Key Duties and Responsibilities:
1. Performs nursing care and other support services to meet the medical and developmental needs of children living in the community anytime during the day, 7 days a week, and 365 days a year. Travel as required.
2. Implements and evaluates nursing care plan for the child; consults and works cooperatively with the family, Resource Ability Nursing Supervisor and other care professionals on the child’s care team. Participates in team meetings as requested.
3. Continuously assesses by observing, measuring, recording and reporting symptoms and changes in child’s condition. Provides other support and tasks that assists the child to live in the community. Escorts the child while in the community. At all times, ensures the safety of the child.
4. Provides guidance to a child and family in health care and treatment. Maintains strong relationship with the family and other members of the health care team. Assists and/or mentors other caregivers or family members.
5. Assists with case management by identifying potential problems and reporting any concerns. Provides input to Resource Ability Nursing Supervisor with regard to the change/ development of appropriate care plans to achieve individual’s desired outcomes.
6. Maintains nursing records and provides reports as required. Maintains related equipment and supplies.
7. Directs and monitors the care provided by unlicensed care providers to ensure the child’s physical and medical needs are met. Delegates nursing functions to other health providers following established agency and professional guidelines as requested.
8. Assists families to obtain the necessary supplies and equipment to ensure continuity of care. Monitors and maintains related equipment and supplies.
9. Provides input into the agency’s continuous quality improvement process as required.
10. Uses sound judgment; carries out job duties in a professional manner and is guided at all times by the agency’s Code of Ethics
11. Performs other related duties as requested
QUALIFICATIONS
Education, Training and Experience
i. Bachelor degree or diploma in Nursing from a recognized School of Nursing
ii. Preferred minimum of three years acute clinical experience with one or more years experience working in a community setting
iii. Preferred three years pediatric experience working with children with complex care needs
iv. Demonstrated ability to work independently within established agency policies and procedures.
v. Demonstrated ability in written, oral and interpersonal communication skills in English.
Employment Requirements
i. Current registration with BCCNP as a practicing nurse
ii. Valid CPR C certificate or better
iii. Good general health and physically and emotionally able to perform the duties of the job
iv. Must sign the Agency’s Statement of Confidentiality and acknowledgement of Agency Abuse Policy
